Registration fees include all meeting materials, luncheon,
continental breakfast, reception, and breaks.

Hotel accommodations are not included in the registration fees.
Each registrant is responsible for his/her accommodation reservation
and payment to the hotel.

Substitutions/Cancellations:

Substitutions are welcome. For full or partial refund, cancellations must be received in writing or by fax before 12:00 noon eastern time on September 10, 1999. After that date, registrants are liable for the full fee.
